You guys are right-- TB is purposefully doing things he knows makes him miserable instead of doing something useful. I think you're missing the bigger picture, though.

TB is trying to distract himself from the reality of his own death looming and the only way he knows how to do that is to be an internet personality. He has to exist on this internet for himself to have any semblance of normalcy and without that he'd fade faster than he'd want to believe he would. This is all he knows. This is the only thing that can keep him going.

His ego likely plays a part but that's not the full truth. The full truth is sadder than that.
