Limitless Heroics: Better worlds via Dice & Disabilities 5e
It's like an expanded version of the famous Combat Wheelchair homebrew thingie, except now with support for different kinds of cripples and retards!
The 2-page preview further reminds me of the Combat Wheelchair thing because it feels the need to justify its existence, and goes further by "explaining" how you can't fix this stuff (at least when you're born this way; 'cause Orcus makes no mistakes).
(One might also argue that getting fixed with magic is "too expensive", but it can't be more expensive than the famed Combat Wheelchair which can float, is effectively indestructible and a magic item that is not affected by anti-magic zones, which I'm pretty sure is artifact-territory).
The preview PDF also has some weird bits...
