Yes, in comparison to your obese body. They're not used to seeing that.

I’m suddenly remembering Patrice O’Neil talking about his asian dry-cleaner telling him they have no word for black person in his language. They’re probably being racist there Goggles.

"Small Face" is actually a Japanese compliment (which I'm sure she is well aware of while posting this). It's an attempt at a humblebrag. But the kids are probably making fun of her.

Everybody in Japan has mistaken her for Shaquille O'neal, and she's been rolling with it.

Sure

Yes. Except I don't believe for a second anyone has actually told her she has a small face.
She was aware of the expression and publicly made the comment "people tell me I have a small face around here, but I don't know what it means..." So that other people would look it up, point it out, and/or at least be aware, that she is thought of as beautiful by Japanese people.
It is a compliment often directed at slim, Western(as in White) women, because European bone structure is less wide than its typical Asian counterpart. Specifically, to have a "small face" means to have an oval-shaped face, with a "V-line" jaw, and fine/delicate features, particularly a long/pointy, small nose. Think "anime-face". Camille Lou, or Li Bingbing are examples of women who would be called small-faced in Japan.
