For a cigar smoker that's really uncommon. Usually oral/mouth cancer, and that's if you smoke several a day.

Either it's a complete random fluke location (like in Breaking Bad), or he's spent years inhaling like a cigarette, which is the wrong fucking way to smoke a cigar.

You puff on it. It's meant to be slowly enjoyed, not chain smoked through. Cigars are wrapped in the leaf, so the nicotine and flavor just go through the gum line and mucosa in the mouth when you hold it there and then breathe out.
You get a lot more for less, which is why often you don't even have to smoke more than half of it.

Cigarettes are made from small strips of unaged tobacco, so they need to draw that shit all the way into the lungs to feel anything.
It's also a tiny amount, so they keep going through them until the whole carton is gone.
One cigar has as much as a whole pack, but because of how it's smoked, 3/4 of users don't have more than 1 a month.
