It's fairly simple: A mech is a walking tank. Anything that looks too impractical to operate within the bounds of suspension of disbelief is shit.
In other words, it needs to look like a somewhat believable utility vehicle of some sorts.
Putting a head on it immediately disqualifies it for that reason. That's not a mech. That's a robot. And like any actual robot that has a head, it's purely there to cater to human emotions and instill a sense of personality for social interactions.
Nobody would ever build a larger than human-scale vehicle shaped like a person for any reason other than social interactions with people.
Certainly no war machine, nor any utility vehicle would ever have a head.
Hands are fine. There's an obvious use or benefit to them for many reasons. Not by any means to hold a large rifle, though. That's yet again absolutely retarded on any scale that is larger than what would just be power armor.

You might like 86 -Eighty Six-. It's like what happens if you make a mecha series centered around that big spider tank from Ghost in the Shell.
It's one of those visual novel animu adaptations though, so it's probably shit and/or doesn't actually have a proper ending.
Though to its credit the klessik choppy 3D animation they use for the mecha kinda works in its favor, since of course you expect a spider tank to not have the smoothest of locomotion.
Metal Gear thought? Rex is definitely a mech despite being described in universe as a “vertical tank” or whatever term they use.
Metal Gear Rex is clearly a mech by any standards.
Super realistic and practical? Perhaps not. But it looks fucking awesome and iconic and has a great name. Plus, it's a nuclear warhead artillery launch platform yet is angled and sleek enough to believably be able to withstand armed resistance it's likely to encounter. A+ rating in my book!
That cockpit is also not what I refer to as a head, nor is it unreasonable for its intended use.
