When you get an internet connection, you have THE internet for all intents and purposes. Some shit may be blocked in your country, which you can work around with a VPN, but it's practically all there. Also, a flat rate means a flat rate.
Knock over NN and not only can the money gouging really start for the reasons you've stated, ISPs would also be open to just go ahead and decide what pages to give you at all.
