Don't you just get it put into your inventory at some point because the game assumes you have it? In OG HL2, even if you skip it due to speedrunning purposes you just have it at some point.

No idea, because the game makes a big deal out of it in a scripted event with Barney IIRC (it's been a few years) that makes it near-impossible to miss unless you're actively trying to for speedrunning as you say.
